Care Coordination

Care coordination is a process that helps members get the services and resources they need to be as healthy as possible. PrimeWest Health care coordinators are nurses, counselors, and social workers. Care coordinators often work closely with county case managers, providing support and assistance. Care coordinators also work directly with members who do not have a county case manager.
