PrimeWest Health Community Health Workers
PrimeWest Health Community Health Workers (CHWs) are part of our Care & Population Health Management team. They are embedded within the communities they serve and have experience connecting people with health and social services. Their role at PrimeWest Health is to make sure members’ health and social needs are met, allowing them to achieve the best possible outcomes. PrimeWest Health CHWs can assist with the following:
- Finding and connecting members with resources such as clothing, shelter, food, and transportation in their communities
- Connecting members to culturally appropriate services for their health and social needs
- Helping members navigate the health care system
- Helping members find programs to assist with finances and things like utility bills
- Helping members set up medical appointments and offering appointment reminders
- Advocating for members’ needs
- Attending medical appointments with members, if requested
- Completing home visits, when appropriate
PrimeWest Health CHWs are currently available in Big Stone, Chippewa, Douglas, Grant, Kandiyohi, Lac qui Parle, Pope, Renville, Stevens, Swift, Traverse, and Yellow Medicine counties.

Brenda believes every person deserves the chance to reach their full potential and live life to the fullest, and she is excited to provide the tools to support each individual’s unique journey. Brenda and her close-knit family live on a hobby farm near Herman, MN. |
