Caregiver Support Program

For Members with Alzheimer's Disease or Related Dementias 

The Caregiver Support Program is designed for PrimeWest Health members with Alzheimer’s disease or related dementias and their caregivers. The program aims to promote the health and well-being of caregivers and members by providing support, coaching, education, and resources. The goal is for the caregiver to feel they can successfully and confidently provide care to the member. To accomplish this, the COACH model is utilized. COACH stands for Create a care plan, Observe the normal routine, Assume a coaching style, Connect tasks with vision and priorities, Highlight efforts with data. PrimeWest Health has developed a set of action cards to help with this.

Components of the Caregiver Support Program include the following:

  • Educating caregivers about Alzheimer’s disease, dementia, and mild cognitive impairment
  • Providing local resources to caregivers and members, including information on support groups or condition-specific caregiver education
  • Addressing social determinants of health (SDOH) which may be affecting quality of life and access to appropriate care
  • Reducing members’ risk of experiencing adverse health events
  • Assisting with self-management skill building
  • Providing Medication Therapy Management (MTM), as appropriate
  • Encouraging ongoing support from members of the clinical practice team, family, friends, and community
  • Connecting members and caregivers with culturally appropriate care

The Caregiver Support Program provides the following: 

  • Individualized coaching and education for members and caregivers
  • Educational materials to help members and their caregivers understand their condition
  • Resources tailored to the needs of the member and their caregiver
  • A free binder to help organize important information
