Health Care Directives

A Health Care Directive is a written document that people can use to inform others of their health care wishes. When someone writes or fills out a Health Care Directive, it allows them to name a person ("agent") to make health care decisions for them if they are unable or unwilling to decide for themselves. A person must be at least 18 years old to make a Health Care Directive. Read the Health Care Directives brochure to learn more.
