Underlying State Initiatives
The Minnesota Department of Human Services (DHS) works closely with partners and providers to improve services and supports for people with disabilities across the state. This work includes implementing reforms where needed, ensuring compliance with Federal standards in an effort to support personal choice and improve lives.
Lead agencies and providers have revised many long-standing practices as a result of Home and Community Based Services (HCBS) reform, which includes the following:
- Statewide Transition Plan to comply with CMS’s HCBS final rule
- HCBS provider self-assessment
- 245D licensing requirements
- HCBS Quality Improvement initiative
- Conversion to a statewide disability waiver rates system
- Elimination of county contracts with HCBS waiver providers
- Focus on positive behavior supports and prohibitions on restraints and seclusion under the Jensen Settlement
- Olmstead Plan activities
- Transition to the person-centered MnCHOICES assessment
- Person-centered care coordination practices based on the DHS-created Person-Centered, Informed Choice and Transition Protocol
