About Person-Centered Care
Person-centered care—sometimes referred to as patient-centered care, patient centeredness, or shared decision making—is a unique, individually focused approach to planning and care delivery for people who are in need of services and supports. It is an important part of empowering PrimeWest Health members and giving them a voice in the planning and decision-making process to actively shape their futures in ways that include their unique values, strengths, preferences, capacities, needs, and desired outcomes or goals.
As a state, Minnesota is moving toward person-centered practices in all areas of service delivery, striving to make sure everyone who receives long-term services and supports (LTSS) and mental health services can live, learn, work, and enjoy life in the most integrated setting.
The information in this section of the website is designed to help PrimeWest Health partners and providers as you interpret and implement person-centered care and shared decision-making practices.
