Five Attributes
Person-centered care includes the following five attributes:
- “Whole-person” care
- Coordination and communication
- Patient support and empowerment
- Ready access
- Autonomy
Person-centered care means much more than simply educating patients about a diagnosis, potential treatment, or healthy behavior, and it does not mean simply giving patients whatever they want. Rather, person-centered care is about the provider and the patient working together. It involves guidance that is provided in the context of full and unbiased information about options, benefits, and risks. It means considering patients’ cultural traditions, personal preferences and values, family situations, social circumstances, and lifestyles as used by the National Academy of Medicine (NAM) and Institute for Healthcare Improvement.
