Bright Futures Program
There's a new baby in your house! Congratulations! Having a new baby at home is exciting and exhausting all at the same time. You’ll likely have a lot of questions ranging from ages and stages of development to what vaccines your child needs. PrimeWest Health wants to help you answer these questions and help you raise a healthy child.
PrimeWest Health has a program called Bright Futures for members from birth through age 21. As part of the program, you will be able to talk with a specially trained nurse who can answer your questions and provide support as you learn to care for your new baby and to help you raise a healthy child. This includes age-appropriate information about healthy children, nutrition, making well-child appointments, arranging transportation, promoting dental care, Public Health Nurse visits, and coordinating care with different providers. This may be especially helpful if this is your first child. The nurse will also be able to help you to follow the plan of care that your health care provider prescribed when your baby was born. As part of this program, you will also get information that will help you take care of you.
You will receive a postpartum (“postpartum” means after pregnancy) packet that includes information about your infant and tips on taking care of yourself. The packet will also include a tool called the Edinburgh Postnatal Depression Scale. You can use this tool to record the way you are feeling as a new mom. It can help identify something called postpartum depression. Many moms experience a form of depression after giving birth. If you are feeling this way, PrimeWest Health wants to help you. Please answer the questions on this tool honestly and return it to us in the envelope provided in the packet.
If you have any questions about Bright Futures, please call PrimeWest Health Member Services at 1-866-431-0801 (TTY users call 1-800-627-3529 or 711. These calls are free.) and ask to talk to the Women & Children Care Coordinator. This information is available in other formats and languages for Minnesota Health Care Programs (MHCP) members. Learn more about how we assure access to care.
