General Health Assessment

To help you identify and understand your personal health risks, PrimeWest Health offers an online health risk assessment (HRA) through WebMD on our Secure Member Site. The Wellness HRA is a series of health‐ and lifestyle‐related questions that takes about 15 minutes to answer. The results of the HRA can help you learn more about your physical and emotional health. In addition, when you fill out the HRA, WebMD will create an individualized action plan with steps you can take to improve your overall well‐being. 

Why complete the HRA?

Here are 4 good reasons to complete the HRA:

1. It’s quick. It should only take about 15 minutes to complete the online questionnaire.
2. It delivers immediate results. Within seconds of completing the questionnaire, you’ll have your results.
3. It provides personalized information. Your HRA results will give you information about your current health status and identify your risk factors. These risk factors often have no symptoms, which means you might not know you have them. Knowing your risk factors may help you make a lifestyle change or seek treatment to avoid serious illness.
4. It is your gateway to new wellness programs. By completing the HRA, you will be eligible to participate in lifestyle and health management programs at no cost. Completing the HRA is also one way PrimeWest Health can determine if you might benefit from participating in our Focused Wellness programs. These are programs that help members manage long-term diseases and are offered at no cost to PrimeWest Health members.

How do I access the HRA?

You can take the HRA by logging in to PrimeWest Health’s Secure Member Site. The Secure Member Site is a health information resource available at no cost to you.

If you have already registered for the Secure Member Site, log in using the same username and password you created when you registered. If you don't remember yout username or password, click on Forgot your username or password? at the bottom of the Secure Member Site Login box.

If you have not yet registered for the Secure Member Site, click Need to sign up? at the bottom of the Secure Member Site Login box. Then, follow the instructions. Once you are registered, you can log in to access the HRA.

Once you are logged in, click on Access Wellness or Complete Health Risk Assessment on the right side of the page under “Quick Links.”

Your answers will not affect your PrimeWest Health benefits. Filling out the HRA is voluntary.
