Comprehensive Diabetes Care (CDC)

Diabetes is a chronic condition that can occur in children and adults. There is no known cure, but there are ways to keep the disease under control. The components of this Healthcare Effectiveness Data and Information Set (HEDIS®)1 measure are as follows:

  • Glycemic Status Assessment for Patients with Diabetes (GSD)
    • Please note this measure was revised and renamed in 2024, and the graph below reflects limited data as a result of this change.
  • Eye Exam for Patients with Diabetes (EED) (retinal eye exam)
  • Blood Pressure Control for Patients with Diabetes (BPD)

Who was measured?
PrimeWest Health members ages 18 – 75 with diabetes (type 1 and type 2) who were continuously enrolled during the measurement year with no more than one 45-day gap in enrollment during the measurement year were measured.

What can providers do?
PrimeWest Health urges providers to recommend these important screenings and help members to understand how to change their lifestyle, diet, and medication as appropriate to get testing levels under control.
