PrimeWest Health offers a Diabetes Focused Wellness program for our members who have diabetes.

What is diabetes?

Diabetes is a chronic condition that causes health problems because glucose (sugar) builds up in the blood. Diabetes can't be cured, but your health care provider can create a treatment plan to help you manage it.

What does this program do?

This program can help you learn how to manage your diabetes and understand why managing it is important. It was designed using the COACH model, which is an innovative way to help people take charge of their health. When you join the Diabetes Focused Wellness program, you will choose a goal to work on to help you meet your needs. As part of our program, a nurse is available to answer your questions. They can help you do the following:

  • Understand the important tests that you should have. These include a dilated eye exam, a yearly comprehensive foot exam, and an HgbA1c blood test.
  • Make healthy lifestyle changes such as eating right and exercising
  • Learn to reduce your risk of heart disease
  • Understand the complications of diabetes
  • Learn how and why to take care of your feet
  • Get supplies and equipment (test strips, glucometers, and syringes)
  • Find care from special providers. These include endocrinologists, diabetes educators, podiatrists, and ophthalmologists.

If you say it’s okay, we may talk to your health care provider about your condition. Your health care provider can help you manage your diabetes. Please talk to them any time you think your diabetes is not under control.

Diabetes Action PlanDevelop an action plan to help with day-to-day management and to know what to do in an emergency.

Diabetes Camp for Kids – PrimeWest Health encourages all eligible members to attend a qualified summer health camp. These camps are a great chance for youth to have fun, learn how to take control of their conditions, and form a community of friends with similar concerns.
