Controlling High Blood Pressure (CBP)

High blood pressure is a serious medical condition that can make the heart work harder than it should. Having high blood pressure increases the chances for heart attack, stroke, kidney failure, heart failure, and blindness. This Healthcare Effectiveness Data and Information Set (HEDIS®)measure looks at the percentage of members who had a diagnosis of high blood pressure and whose blood pressure was adequately controlled during the measurement year.

Who was measured?
PrimeWest Health members ages 18 – 85 who were continuously enrolled during the measurement year with no more than one 45-day gap in enrollment and who had a diagnosis of high blood pressure during the first six months of the measurement year were measured.

What can providers do?
Providers can help increase rates by encouraging follow-up visits for blood pressure monitoring and by providing education about diet and exercise.
