Care for Older Adults (COA)
This Healthcare Effectiveness Data and Information Set (HEDIS®)1 measure addresses the following two important areas for older adults:
- Functional status assessment
- Medication review
Who was measured?
PrimeWest Senior Health Complete (HMO SNP) members age 66 and over as of December 31 of the measurement year who were continuously enrolled with no more than one 45-day gap in enrollment during the measurement year were measured.
Our results are shown in the graph below. Also shown is the state average and the 4-star rate as determined by the Centers for Medicare & Medicaid Services (CMS) for PrimeWest Senior Health Complete. Every year Medicare evaluates plans based on a 5-star rating system.
What can providers do?
Providers can help improve this measure by thoroughly documenting when medications are reviewed and discussing members’ functional status for activities of daily living and/or instrumental activities of daily living (ADLs/IADLs).
