Medication Therapy Management (MTM)
PrimeWest Health offers Medication Therapy Management (MTM) programs that can help members who take multiple medications to treat multiple chronic conditions and members with very high drug costs. The programs can help make sure that our members are using the drugs that work best to treat their medical conditions. They also help us identify possible medication errors. MTM programs are voluntary and free for members. A team of pharmacists and doctors developed the programs for us.
We have a couple of different MTM programs. The MTM program you may participate in is determined by your plan membership. Choose one of the links below to learn more about our MTM programs.
- MTM program for members of PrimeWest Senior Health Complete (HMO SNP) and Prime Health Complete (HMO SNP)
- MTM program for members of Families and Children1, MinnesotaCare, Minnesota Senior Care Plus (MSC+), and Special Needs BasicCare (SNBC)
