Chronic Obstructive Pulmonary Disease (COPD)
We offer a Chronic Obstructive Pulmonary Disease (COPD) Focused Wellness program for our members who have COPD.
What is COPD?
COPD is a narrowing of the airways. This leads to a restriction of the flow of air to and from the lungs, causing shortness of breath.
What does this program do?
This program can help you learn how to manage your COPD and understand why managing it is important. It was designed using a model called COACH, which is an innovative way to help people take charge of their health. When you join the COPD Focused Wellness program, you will choose a goal to work on to help you meet your needs. As part of our program, a nurse is available to answer your questions. They can help you do the following:
- Understand your COPD
- Understand your COPD medicines
- Get the supplies and equipment you need
- Find and receive care from special providers who work with COPD
If you say it’s okay, we may talk to your health care provider about your condition. Your health care provider can help you with your COPD. Talk to them any time you think your COPD is not under control.
COPD Action Plan – Develop an action plan to help with day-to-day management and to know what to do in an emergency.
