Blood Pressure Control for Patients with Diabetes (BPD)

Monitoring and controlling high blood pressure is an important part of managing diabetes.

Who was measured?
PrimeWest Health members ages 18 – 75 with diabetes (type 1 and type 2) who were continuously enrolled during the measurement year with no more than one 45-day gap in enrollment during the measurement year were measured.

Please note that this measure is in the transition period to be moved to a different form of reporting for Healthcare Effectiveness Data and Information Set (HEDIS®)1 called Electronic Clinical Data Systems (ECDS).

Our results are shown in the graph below. Also shown is the State average for all programs; the national average for Families and Children (F&C), MinnesotaCare (MnCare), and Special Needs BasicCare (SNBC) Medical Assistance-only; and the 4-star rate determined by the Centers for Medicare & Medicaid Services (CMS) for our integrated Minnesota Senior Health Options (MSHO) program, called PrimeWest Senior Health Complete (HMO SNP), and our integrated SNBC program, called Prime Health Complete (HMO SNP). Every year Medicare evaluates plans based on a 5-star rating system.
