All corrected claims require an Adjustment Request Form

Effective Monday, August 25, 2008, please note that any corrected claims will now require an Adjustment Request Form accompanying the corrected claim. Corrected claims requiring this form include any claims that have previously been submitted and have received a full payment, a partial payment/denial, or a total claim denial. You will need to include an Adjustment Request Form in any instance when you are correcting the original claim submission. The following includes a few situations when you will need to include the Adjustment Request Form, but should not be read as an exhaustive list:

• When one line of the claim was denied due to an issue with a modifier
• When the entire claim was denied for primary Explanation of Benefits (EOB)
• When the entire claim was paid, but you later realize it was submitted with the incorrect practitioner
• When a line of the claim was paid, but you later realize it was billed in error

The Adjustment Request Form found on our website is an "interactive" form. This means that you have the option to type the information into the form before printing. You may also save the typed information by selecting File>Save As and saving the completed document to your computer.

Comprehensive Elder Health Evaluation (CEHE) incentive

The Minnesota Department of Human Services (DHS) recently announced the implementation of a Comprehensive Elder Health Evaluation (CEHE) incentive payment for primary care providers.

PrimeWest Health would like to ensure that primary care providers who are currently contracted with us and working in our communities are aware of this opportunity.
