Claim Submission Requirements:  Effective with service dates on or after May 1, 2006 all claims submitted for services rendered by a PCA shall have the professional information on the claim and their professional identification number in Box 24k on the CMS-1500 claim form.  Claims submitted without this information will be denied as "Attending Provider Required" (denial code 119).

For clarification: The attending professional is the personal care attendant, and the professional ID is their provider ID number issued by the State of Minnesota, Department of Human Services (DHS).

A valid diagnosis code is also required for PCA services, consistent with MN DHS billing requirements. (06/06/06)
