Diabetes Disease Management program

PrimeWest Health offers a Diabetes Disease Management program for our members ages 18 - 64 with diabetes.

 

What is diabetes?

Diabetes is a chronic disease that causes health problem because glucose (sugar) builds up in the blood. Diabetes can't be cured, but your health care provider can create a treatment plan to help you manage it.

 

What does this program do?
When you join the Diabetes Disease Management program, you will get helpful information about diabetes several times a year. As part of our program, a Diabetes Disease Management nurse is available to answer your questions. He/she can give you information about the following:

  • Daily foot care
  • Important tests that you should have. These include a dilated eye exam, a yearly comprehensive foot exam, and an HgbA1c blood test.
  • Nutrition and exercise
  • Lowering your risk of heart disease
  • The complications of diabetes

The nurse can also help you do the following:

• Get supplies and equipment (test strips, glucometers, and syringes)
• Find care from special providers. These include endocrinologists, diabetes educators, podiatrists, and ophthalmologists.

We may also let your health care provider know about your diabetes. Your health care provider can help you manage your diabetes. Please talk to him/her any time you think your diabetes is not under control.
