Asthma Disease Management program
We offer an Asthma Disease Management program for our members ages 3 - 21 with asthma.
What is asthma?
Asthma is chronic swelling of the airways. It may seem to go away, but the swelling can flare up at any time.
What does this program do?
This program can teach you how to manage your asthma. It can also tell you why managing your asthma is important. When you join the Asthma Disease Management program, you will get helpful information about asthma several times a year. As part of our program, an Asthma Disease Management nurse is available to answer your questions. He/she can help you do the following:
- Understand your asthma
- Understand your asthma medicines
- Get the supplies and equipment you need
- Find and receive care from special providers who work with asthma
We may also let your health care provider know about your asthma. Your health care provider can help you with your asthma. Talk to him/her anytime you think your asthma is out of control.
