Disease Management Programs

PrimeWest Health's Disease Management programs are educational programs for people with chronic (long-term) diseases. The purpose of Disease Management is to help members manage their disease(s).

If you are a member who can benefit from Disease Management, a nurse will call you to talk about your disease. The nurse will also send you informational mailings. He/she may also offer daily self-care tips to help manage your disease and help you set health care goals.
