Welcome to PrimeWest Health System's MSC Plus program!
This program is designed especially for people age 65 years and older. While you are enrolled in PrimeWest's Minnesota Senior Care Plus program, you will get great customer service, easy access to health care services, and personal one-to-one interaction with a PrimeWest Care Coordinator. Your PrimeWest Care Coordinator will work closely with your County Case Manager to help you maintain your health while living in your own home.
