Prime West Health System - Neighbor carying for Neighbor

MinnesotaCare is a subsidized health insurance program for Minnesota residents who do not have access to affordable health care coverage. PrimeWest MinnesotaCare provides services for adults who meet MinnesotaCare eligibility requirements and have paid the required premium.

PrimeWest Medical Assistance and General Assistance Medical Care programs provide health care coverage for low income residents in the PrimeWest counties.

Medicare Part D is the new prescription drug coverage program that is available for PrimeWest‘s members who are over age 65 and eligible for Medicare parts A and B. Part D coverage will start Jan. 1, 2006 for people  enrolled in MSHO.

This program is designed especially for people on Medical Assistance, between the ages of 18 and 64, certified disabled and living in Big Stone, Douglas, Grant, McLeod, Meeker, Pipestone, Pope, Stevens, Traverse or Renville Counties. PrimeWest Health SNBC members have access to the same benefits covered under fee-for-service plus additional benefits. Your PrimeWest Health Care Coordinator will work closely with your County Case Manager to help you maintain your health while living in the community.
