MinnesotaCare (MnCare)

PrimeWest Health MSC provides coverage for health care (hospital, clinical, and preventive care; prescription drugs; dental care; and medical equipment) for people 65 and over enrolled in Medical Assistance (MA), General Assistance Medical Care (GAMC), and MinnesotaCare.
Minnesota Senior Health Options (MSHO)
PrimeWest Health MSHO is a health care program for seniors age 65 and over who are eligible for Medicaid and Medicare. People only on Medicaid can also join. Through MSHO, the health care and support services seniors need are coordinated to make it easier for people to get these services. In MSHO, PrimeWest Health assigns a Care Coordinator to each enrollee to help with paperwork and getting health care and support services. Seniors can choose to join MSHO or stay in the Minnesota Senior Care Plus program. There is no additional cost to join MSHO.
Special Needs BasicCare (SNBC)
This program is designed especially for people on Medical Assistance, between the ages of 18 and 64, certified disabled and living in Big Stone, Douglas, Grant, McLeod, Meeker, Pipestone, Pope, Stevens, Traverse or Renville counties. PrimeWest Health SNBC members have access to the same benefits covered under fee-for-service plus additional benefits. Your PrimeWest Health Care Coordinator will work closely with your County Case Manager to help you maintain your health while living in the community. PrimeWest Health's goal is to support you and keep you healthy.
PrimeWest Health has surveyed their enrolled providers regarding accessibility for persons with disabilities. SNBC members may call the dedicated SNBC Member Service's line at 1-877-600-4913 (toll free) to inquire about specific accessibility information for a primary care provider that was obtained through PrimeWest Health's survey regarding physical accessibility.
