Contracting & Credentialing

PrimeWest Health's participating providers are facilities that have entered into a contract with PrimeWest Health and are part of our network.

The PrimeWest Health network consists of many provider types and specialties. If your facility is not currently in the PrimeWest Health network, please read the non-participating provider information below.

Non-participating Providers
Most services require a Service Authorization for non-participating providers. Please review the Service Authorization information section. If you would like to be considered for contracting with PrimeWest Health, please complete the Participation Request form.

Credentialing is not required for non-participating providers only.

Participating Providers
If you are a participating provider, please complete Contracting Forms for participating providers as applicable or as requested by PrimeWest Health.
