Introduction

Welcome to PrimeWest Health! We give our members easy access to health care services and providers and great customer service. We provide comprehensive coverage for health care services, including preventive care, hospitalization, dental care, eye care, mental health and chemical dependency services, chiropractic services, and prescription drugs. We are owned by the 13 rural counties we serve and our main office is located in Alexandria, Minnesota. Click here to see our member counties.

We offer the following programs for people who are eligible (click on any program below to learn more about who is eligible and how to enroll):

Prepaid Medical Assistance Program (PMAP)
For children, pregnant women, and adults under age 65 who have Medical Assistance (Medicaid)

MinnesotaCare
For children, pregnant women, and adults who are eligible for the State MinnesotaCare program. Members pay a monthly premium to the State.

Minnesota Senior Care Plus (MSC+)
For people age 65 or over who have Medical Assistance (Medicaid) and, if they have Medicare, have it through an insurer other than PrimeWest Health

Special Needs BasicCare (SNBC)
For people age 65 or over who have Medical Assistance (Medicaid) and, if they have Medicare, have it through an insurer other than PrimeWest Health

PrimeWest Senior Health Complete (HMO SNP)
This is a Minnesota Senior Health Options (MSHO) program for people age 65 or over who choose to have their Medical Assistance (Medicaid) and Medicare integrated through PrimeWest Health

Prime Health Complete (HMO SNP)
This is a Special Needs BasicCare (SNBC) program for people who have a certified disability, are ages 18 – 64, and choose to have their Medical Assistance (Medicaid) and Medicare integrated through PrimeWest Health
