Prime Health Complete (SNBC)
Prime Health Complete (SNBC) is a Minnesota Special Needs BasicCare (SNBC) program for people with disabilities, ages 18 – 64, who are on Medical Assistance (MA). While you are in Prime Health Complete (SNBC), you will get great customer service and easy access to health care services. You will also work with a county case manager. Your county case manager can help you arrange health care and support services. County case managers and PrimeWest Health care coordinators work together to help you maintain your health whether you live in your own home or in a care facility.
To qualify for Prime Health Complete (SNBC), you must meet the following criteria:
- You are at least 18 years of age and under age 65
- You qualify for Medical Assistance (MA).To learn more about Medical Assistance and how you qualify, visit the Minnesota Department of Human Services (DHS) website
- You have a certified disability through the Social Security Administration or the State Medical Review Team; or you have been determined by the county to have a developmental disability (DD) for purposes of the DD waiver
Note: If you meet these eligibility criteria and also have Medicare, read about our Prime Health Complete (HMO) program for members with Medicare.

- Read your Primary Care Network Listing to see a list of primary care clinics in the PrimeWest Health network.
- Read your Provider Directory to see a full list of health care providers (including specialists) in the PrimeWest Health network.
- Use our online search feature to see if a specific provider is in the PrimeWest Health network.
- To see if specific prescription drugs are covered by Prime Health Complete (SNBC), use our online search feature. Note: If you have other insurance coverage, contact that insurance plan for information about its drug coverage. If you have Medicare, you must enroll in a Medicare Prescription Drug Plan (Part D) to receive most of your prescription drug services. PrimeWest Health will not pay for any Part D eligible drugs.
