Use these self-care guides at home to help you do the following:
- Decide when to call a health care provider or 911 about your symptoms
- Treat your symptoms at home
- Prepare for a health care visit
- Make wise decisions about tests, medicines, and surgeries
- Manage chronic health problems
- Get better care at lower costs
- Learn how to prevent health problems
These guides do not replace your health care provider, but they can help you get more out of your health care visits. Use the books to prepare questions for your health care provider to learn more about your health, treatment options, tests, or medicines.
There are also areas in this book that contain "Go-to-Web" codes that allow you to access further information on the web. Type the code in the search box below (codes look like this: p502) for more information on a particular topic.

The nurses can also help you decide whether your condition is something you can take care of at home, or if you need to make an appointment with your health care provider or even go to an emergency room. When you need this kind of help, call the nurse line. The phone number is on the back of your membership card.
If you need help finding a provider, want to know if a procedure is covered, or have eligibility questions, please DO NOT call the nurse line. Nurses at the nurse line offer general health information and do not have information specific to PrimeWest Health members. You can call PrimeWest Health's Member Services at 1-866-431-0801 (toll free) to get answers to these kinds of questions.

Our General Health Assessments are health risk assessment (HRA) questionnaires for members ages 18 – 64. We use them to provide members with an evaluation of their health risks and quality of life. We have one General Health Assessment for men and one for women.
To complete the assessment, click on the link below and choose the appropriate one (for men or for women). Print the assessment, fill it out, and return it to:

You may also call Member Services at 1-866-431-0801 (toll free) to have a pre-addressed, stamped envelope sent to your home. Or, you may complete the assessment over the telephone by contacting Nicole at 1-888-762-2951 (toll free).
Once you have completed the assessment, we use the answers in two ways:
- You will receive a Personal Wellness Report.
- We also use the information to decide if any of our health education and support services would be helpful for you. If so, we will contact you with more information.
Your answers will not affect your PrimeWest Health benefits in any way. Filling out the assessment is voluntary. We will keep your answers confidential and will not share them with anyone.
