Heart Health/Optimal Vascular Care (OVC) Disease Management Program
We offer an Optimal Vascular Care (OVC) Disease Management program for our members who are ages 18 – 75 and have had an acute myocardial infarction (AMI), coronary artery bypass graft (CABG), percutaneous transluminal coronary angioplasty (PTCA), or ischemic vascular disease.
What is OVC?
According to the American Heart Association, heart disease is the number one killer of men and women. It is among the top five most common chronic disabilities. The OVC Disease Management program is an educational program that provides a long-term approach to managing heart disease.
What does this program do?
This program can help you and your primary care provider manage your heart disease. As part of this program, a Disease Management nurse is available to answer your questions. He/she can provide education and support to make lifestyle changes. He/she can also help you do the following:
- Improve your quality of life and health status
- Reduce your risk of experiencing adverse cardiovascular events
- Build skills to change and sustain the behaviors that influence controllable risk factors
We may also let your health care provider know about your heart disease. Your health care provider can help you manage your heart disease. Please talk to him/her any time you think your heart disease is not under control.
