Disease Management Programs/Chronic Care Improvement Programs (DM/CCIP)
PrimeWest Health's Disease Management/Chronic Care Improvement Programs (DM/CCIP) are educational programs for people with chronic (long-term) diseases. The purpose of the DM/CCIP is to help members manage their disease(s).
If you are a member who can benefit from a DM/CCIP, a nurse will call you to talk about your disease. The nurse will also send you informational mailings. He/she may also offer daily self-care tips to help manage your disease and help you set health care goals.
PrimeWest Health offers the following DM/CCIPs (click on the program names below to learn more):
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
- Depression
- Heart Health/Optimal Vascular Care (OVC)
