Disability access information for primary care clinics

PrimeWest Health gathers disability access information by sending a survey to our network primary care clinics. Each clinic was asked questions about specific types of disability access. This information was current as of September 2010. Click on a question below to see results for each clinic. Call Member Services at 1-866-431-0801 (toll free) if you don't see your clinic or have questions about whether a clinic is accessible for you.

How many handicap-accessible parking spaces are available?
Is assistance available to call for transportation?
Is there an automatic door opener that keeps door open with sufficient time to allow wheelchair entrance?
Does your clinic have TTY equipment?
At the time of scheduling a new patient appointment, does your clinic ask if the patient has special needs or requires special accommodations?
Are examination/treatment tables adjustable to wheelchair level?
Are scales designed to weigh patients with physical disabilities?
Can your scales weigh people who are more than 350 pounds?
Do your waiting areas accommodate wheelchairs?
Can your clinic accommodate a patient who needs a private waiting area?
Are there handrails in your hallways?
Are your bathroom and accessibility signs in Braille?
Do you have a 5-foot wheelchair turning radius in at least one bathroom stall?
Can door handles be operated with a closed fist?
Is appropriate transfer assistance to exam tables, lab, mammogram, or X-ray equipment available (e.g., transfer lift equipment and staff assistance)?
Do elevators have visual and audible signals?
Is your reception desk at chair level?
Is public transportation available to your clinic?
How frequently does the clinic staff receive cultural competency training?
Is your clinic currently working on any specific cultural programs or initiatives to address disability needs in your health care setting such as disability awareness, disability culture, aging with a disability, or youths with disabilities?
How often, if at all, does your staff receive training on caring for patients with physical disabilities, developmental disabilities,or behavioral health challenges?
