Compliance Intake Form
The Compliance Intake Form is used by PrimeWest Health's Compliance department to gather information regarding suspected misconduct. Providers, members, and employees should use the form to report any suspected internal or external fraud, waste, or abuse. After the form is completed, fax it to Rebecca Fuller, Corporate Compliance Officer, at 1-320-762-8750.
