Chronic Obstructive Pulmonary Disease (COPD) Disease Management/Chronic Care Improvement Program (DM/CCIP)
We offer a Chronic Obstructive Pulmonary Disease (COPD) Disease Management/Chronic Care Improvement Program (DM/CCIP) for our members who have COPD.
What is COPD?
COPD is a narrowing of the airways. This leads to a restriction of the flow of air to and from the lungs, causing shortness of breath.
What does this program do?
This program can teach you how to manage your COPD. It can also tell you why managing your COPD is important. When you join the COPD DM/CCIP, you will get helpful information about COPD several times a year. As part of our program, a COPD DM/CCIP nurse is available to answer your questions. He/she can help you do the following:
- Understand your COPD
- Understand your COPD medicines
- Get the supplies and equipment you need
- Find and receive care from special providers who work with COPD
We may also let your health care provider know about your COPD. Your health care provider can help you with your COPD. Talk to him/her any time you think your COPD is not under control.
